 來說說最近我的進度好了，提供給一些也是自己申請的朋友作參考

1.衛生署申請文件方面：同時要填寫醫事人員請領英文證明申請書【第一份500,第二份以上200】及求證醫事人員資格申請書（CG- Request for Validation of Registration/License for Certification Program）向衛生署醫事處申請，工作天為6天，最好是再打電話跟他確認，他會詢問你的身分證字號，然後電話等很久後會告訴你啥時已寄出。

只要填寫好"衛生署申請書"的"郵寄地址"他就可以幫我們寄出
另外註明需要一起寄出的文件

2.指紋卡部分：我是還沒寄任何報名表的情況下先以e-mail方式（webmasterbrn@dca.ca.gov）跟他們要求指紋卡，過了十天就收到了，然後再到”戶籍”所在的刑事警察局去蓋，all free！像我戶籍在桃園縣，就是到縣政府旁的刑事警察局（蓋的時候還遇到帶手銬來蓋的...）喔!記得提醒他不要折到指紋卡喔

2.singnature of offical taking fingerprints, 承辦人員應該都有經驗,蓋中文章即可,只要提醒他不要折到卡就好了,千萬不可折到!!
3.日期我當時是寫,DEC.27.2004.

這裡其實有爭議
有人建議等申請文件送出後等CA RN自己寄指紋卡來
有人建議是像我這樣先請CA RN寄指紋卡來..
看你們啦
兩種方法都可以
只是我會選先拿指紋卡是因為,覺得比較省時間,而且跟申請文件一起送件,比較不容易被他們搞丟.不論是CG或NCLEX-RN都很容易把文件弄丟..Even同個包裹裡的其他文件他說有收到....見鬼了...可是沒辦法,遇到這種情形,只好乖乖重新準備文件補寄啦,不然也沒辦法

黃色圈起來的地方：是主選單,各種預使用的服務都從這選
紅色字的地方：是CG的新改革,在使用者一登錄後就可以看到歡迎畫面,有自己的英文名字及照片,而我的沒有照片是因為CG很天才,我都通過考試了,還告訴我說沒收到我的照片...明明人家都寄在同一份包裹裡.....現在打算等Visa Screen時再一起補寄
紫色字下面的文字：提醒你在等下的報名程序中,你所需要準備好的東西有哪些


先介紹如何下order(如何選擇要報名的項目)
按左手邊的Place Order
然後按右下角的Begin Selection Process


接下來就可以選擇要的Service是 CP or Visa Screen, 然後各自進入右下角的Next


以下為報名CP時的一些填寫項目及注意事項：

Step 4: Education Overview

分為a)secondary education:
Secondary school education, sometimes called "high school" or "preparatory school," generally lasts between three and six years. In some countries, secondary education is divided into lower and upper cycles.
所以我這一項應該是填寫國中,是吧.
Usually, students study a common curriculum in the early years, and more specialized courses in the later years. Completion of secondary school results in a diploma or leaving certificate.

b)Post-Secondary Non-Professional Education:
This type or category of school provides higher education, such as a college or university, which is not related to the healthcare profession. Graduation from a secondary school and entrance exams are often required to enter these schools. Technical or vocational schools may be included in this category, if the training you received is not related to healthcare. Usually, graduation from this type of school results in a certificate or degree.
所以這是指非護理的college or university學位

c)Professional Schools
Professional education includes colleges, universities, technical and vocational education -- schools that prepare you for a profession in healthcare. Successful completion of professional higher education normally results in a certificate, degree, diploma, or license.
那我把護校跟護理學院的資料填這

MACKAY SCHOOL OF NURSING ,
NATIONAL TAIPEI COLLEGE OF NURSING...ㄏㄏ...很多學校都有選項喔
原來這麼多前輩來自台灣學府阿..

點選Taiwan後出現了Registered Professional Nurse

當時就2005年七月跟十一月兩場可選

喔喔...我都還沒有..直接給他按NEXT摟....

我發誓....這些都是真的.....布拉布拉的..
恩..很誠心的按下YES→NEXT

重新看一次有沒有要修改的資料

就是把你剛剛填好的資料印下來存檔都可以,
然後交給該寄出的寄出
(發現Step 4: Education Overview填對了)

Step 13: Application Review
to choose your payment options and to submit your order.

除了學校及衛生署外
自己需寄出的是不是
1.PHOTO IDENTIFICATION CARD 貼上照片一張
2.國中高中英文畢業證書
3.由於我採線上刷卡,匯票不用
*4.另外照片兩張(包含原本需貼在CERETIFICATION OF ACCURACY的那一張)

不過也有朋友只寄一張…所以這不確定,不過都可以考試
